Prints:
If you need full size paper prints you can order them from CPM for $1.00 per page. You can look through the set online or in our office and order the pages you need. Call or send us an email or fax indicating which pages you want printed. You can pay with a check or cash when you pick them up at our office.

Pre-Qualification Form:
Pre-Qualification Form-Click here to download the form to provide information we need from subcontractors that we have not used in the past. This will help us as we consider your company for a possible contract to do work for us.
Preliminary Paperwork:
Subcontractor Forms-Click here to download the forms you need to fill out to work on a CPM project. These forms must be turned in before you start on a job and before you will receive any payments for your work.
Billing Forms:
Progress Payment Billing Form - Click here to download a billing form that you can in Microsoft Excel or Open Office. You can then fill in on your computer. It will do all the calculations for you. You should be able to save it to your computer and email it to us. (If you get a choice between "Open" or "Save" choose "Save".)
Billing Form Online - If you do not have Excel or OpenOffice you can use this online form to create your bill it will also do the calculations for you. Please only type in the colored cells. If there are numbers in cells just delete them before you start. You can then email it to us. When you are done, delete what you have put in the colored cells.
When printing the form choose "Actual Size" and "Portrait". If you want create a PDF that you can email, choose "Print to PDF" as your printer destination.
